ABSTRACT

OBJECTIVE To introduce the drug diagnosis, treatment plan and pharmaceutical care of a case of central nervous system infection caused by Mycobacterium malmoense combined with drug-induced liver injury, and to provide a reference for clinical pharmacists to carry out pharmaceutical practice. METHODS Clinical pharmacists participated in the whole treatment of a case of central nervous system infection caused by M. malmoense combined with drug-induced liver injury. Considering the patient’s headache worsened and the skin appeared red spot again, clinical pharmacists suggested stopping isoniazid and adjusted the quadruple anti-infection regimen to Rifampicin capsules+Ethambutol hydrochloride tablets+Amikacin sulfate injection+ Clarithromycin tablets, and sending cerebrospinal fluid samples for the next-generation sequencing to identify the pathogen. After the pathogen was identified as M. malmoense, clinical pharmacists recommend continuing the above quadruple regimen. When the patient suffered from drug-induced liver injury, clinical pharmacists assisted physicians to adjust the medication plan, and suggested that Rifampicin capsules should be discontinued, Moxifloxacin hydrochloride tablets should be used for anti-infection treatment, Glutathione tablets should be used for liver protection treatment, and renal function and electrocardiogram monitoring should be performed. RESULTS & CONCLUSIONS The physicians adopted the advice of clinical pharmacists, and the patient was discharged after the condition improved. Clinical pharmacists review the patient’s medical and medication history, consult guidelines/consensus, research reports, and other literature materials, analyze the causes of adverse reactions based on the patient’s condition and the characteristics of drug action, adjust the medication plan of anti-infective drugs in a timely manner, and provide targeted treatment for adverse reactions. From a pharmaceutical perspective, they assist physicians in improving clinical treatment decisions and ensuring the efficacy and safety of clinical medication.

ABSTRACT

Oral lichen planus (OLP) is a common chronic inflammatory disease of the oral mucosa. The prevalence rate of OLP in adults is 0.5%-2%. The etiology and pathogenesis of OLP are still unclear. The pathogenesis of OLP may be related to the genetic polymorphism of some genes. Currently, the gene families, including tumor necrosis factor, interferon, interleukin, enzyme, and receptor, have been extensively studied. This work reviews related studies on gene polymorphism of OLP.

ABSTRACT

Objective To adopt the expression profile chip to investigate the effect of Rabdosia serra (Maxim.) hara water extract on related gene of human hepatocellular carcinoma(HCC) HepG2 cells for researching the possible mechanism of its water extract on HCC.Methods The human HCC HepG2 cells were cultured in vitro. After adding Rabdosia serra (Maxim.) hara water extract (9.54 mg/mL) for 24 h action, the expression profile chip was adopted to detect the HepG2 gene change after Rabdosia serra (Maxim.) hara action and then the chip results were verified by using RT-PCR and Western blot. Results The phase contrast microscope observation found that compared with the negative control group, the number of HepG2 cells was significantly reduced after Rabdosia serra (Maxim.) hara water extract action. The expression profile results showed that after Rabdosia serra (Maxim.)hara water extract action for 24 h,264 genes were risen to over twice folds compared with the negative control group and 194 genes were decreased by over twice folds compared with the negative control group. The gene ontology(GO) and KEGG analysis indicated that Rabdosia serra (Maxim.) hara could up-regulate multiple genes in DUSPs and IGFBPs family and down-regulate multiple genes in MCMs family. The RT-PCR detection found that compared with the negative control group,DUSP1 and IGFBP1 in the Rabdosia serra (Maxim.) hara treatment group were increased, while FXR and ALDH8A1 were decreased (P<0.01),which were consistent with the results of expression profile chip. The Western blot results found that the protein expression level of DUSP1 in the Rabdosia serra (Maxim.) hara treatment group was also significantly higher than that in the negative control group (P< 0.01).Conclusion The expression profile chip shows that Rabdosia serra (Maxim.) hara can inhibit the proliferation of HCC cells by regulating multiple genes.

ABSTRACT

AIM:To investigate the role of prostaglandin E2receptor 2 agonist (EP2A) in proliferation and homing of human CD34 +cells. METHODS:Bone marrow fluid and peripheral blood containing stem cells were collected from healthy donors mobilized by granulocyte colony-stimulating factor in our department. Human CD34 +cells were isolated by the method of magnetic-activated cell sorting microbeads. Bone marrow mononuclear cells were isolated by Ficoll-Paque centrifugation, and the bone marrow mesenchymal stem cells(BMMSC) were cultured with L-DMEM. Human CD34 +cells and BMMSC were divided into 4 groups, and treated with PGE2(as positive control),DMSO(as negative control),EP2A and EP2A+prostaglandin E2receptor 2 antagonist (EP2AA),respectively. After exposed to the reagents, human CD34 +cell viability was measured by CCK-8 assay, the number of colonies was evaluated by colony-formation assay, the cell cycle distribution was analyzed by flow cytometry, and the protein expression of survivin,β-catenin and CXC chemokine receptor 4 (CXCR4) was detrmined by Western blot. Moreover, the concentration of stromal cell-derived factor-1α (SDF-1α) in the BMMSC was detected by ELISA. RESULTS:The cell viability and the colony number of human CD34 +cells in EP2A group were not higher than those in negative control group. Furthermore, the proportion of human CD34 +cells treated with EP2A in G2/M phase was not elevated compared with negative control group. The protein expression of survivin and β-cate-nin did not up-regulated in human CD34 +cells exposed to EP2A,but the protein expression of CXCR4 in human CD34 +cells and the concentration of SDF-1α in BMMSC were elevated. CONCLUSION:EP2A promotes human CD34 +cell homing in vitro but not proliferation.

ABSTRACT

OBJECTIVE: To investigate the changes of incidence and prognosis of epithelial ovarian cancer in thirty years in Taiwan. METHODS: The databases of women with epithelial ovarian cancer during the period from 1979 to 2008 were retrieved from the National Cancer Registration System of Taiwan. The incidence and prognosis of these patients were analyzed. RESULTS: Totally 9,491 patients were included in the study. The age-adjusted incidences of epithelial ovarian cancer were 1.01, 1.37, 2.37, 3.24, 4.18, and 6.33 per 100,000 person-years, respectively, in every 5-year period from 1979 to 2008. The age-specific incidence rates increased especially in serous, endometrioid and clear cell carcinoma, and the age of diagnosis decreased from sixty to fifty years old in the three decades. Patients with mucinous, endometrioid, or clear cell carcinoma had better long-term survival than patients with serous carcinoma (log rank test, p<0.001). Patients with undifferentiated carcinoma or carcinosarcoma had poorer survival than those with serous carcinoma (log rank test, p<0.001). The mortality risk of age at diagnosis of 30-39 was significantly higher than that of age of 70 years or more (test for trend, p<0.001). The mortality risk decreased from the period of 1996-1999 (hazard ratio [HR], 0.90; p=0.054) to the period after 2000 (HR, 0.74; p<0.001) as compared with that from the period of 1991-1995. CONCLUSION: An increasing incidence and decreasing age of diagnosis in epithelial ovarian cancer patients were noted. Histological type, age of diagnosis, and treatment period were important prognostic factors for epithelial ovarian carcinoma.

ABSTRACT

BACKGROUNDThere is a relatively high failure rate of the femoral component in patients with avascular necrosis at the intermediate-term follow-up. Improving the geometrical fit of the femoral stem against the medullary canal may help to provide long-term survivorship of the hip replacement for patients with avascular necrosis.

METHODSWe designed a specific stem, based on morphometric studies of proximal femoral canals in Chinese avascular necrosis patients and evaluated the stem by finite element analyses, comparing the novel stem with two commercially available and commonly used stems.

RESULTSThe morphometric data from avascular necrosis patients showed specific geometric differences in the proximal femoral canal, including profile curves in both the sagittal and coronary planes than the patients with femoral neck fracture. The shorter stemmed prostheses (Fitmore(®) and our stem) performed better than the longer stemmed prosthesis (VerSys(®)).

CONCLUSIONSThis is the first study to investigate the femoral geometries of Chinese avascular necrosis patients. Our stem provides better stability and is theoretically beneficial to bone ingrowth, which may increase the long-term stability and fixation of the implant.

ABSTRACT

OBJECTIVETo observe the antiulcer effects of pomegranate tannins in animal models.

METHODGastric ulcer models were established by pylorus ligation, intragastric absolute ethanol, and water-immersion stress, respectively. The ulcer index, the contents of nitric oxide (NO) and malondialdehyde (MDA), the activities of glutathione peroxidase (GSH-PX) and superoxide dismutase (SOD) from gastric mucosa of rats, the gastric juice volume, free acidity, total acidity, total acid output, the pepsin activity, the amount of adherent mucus and free mucus were measured, respectively.

RESULTPomegranate tannins (500, 150, 50 mg x kg(-1)) significantly inhibited ulcerative formation induced by both water immersion stress and pylorus ligation, obviously decreased the gastric mucosa damages induced by intragastric absolute ethanol, in dose-dependent manner. Pomegranate tannins significantly inhibited absolute alcohol-induced elevation of MDA as well as decreasing of NO level, and activities of both SOD and GHS-PX from gastric mucosa. Pomegranate tannins significantly increased the secretion of adherent mucus and free mucus, but did not affect elevation of the free acidity, total acidity, and total acid output, gastric juice volume, gastric pepsin activity induced by pylorus ligation.

CONCLUSIONPomegranate tannins play a protective role against gastric ulcer. Its antiulcer effect is related to increasing secretion of adherent mucus and free mucus from the stomach wall, which may inhibit generation of oxygen-derived free radicals, and decrease the consumption of GSH-PX and SOD, and maintain content of NO at normal level.
